This Ask reminded me of something I did for work a few years ago, have given talks about a few times, and recently turned into open source documents. They're meant help create a better workplace by helping people understand how to escalate problems. Prerequisite: A company that wants a better workplace.
I'll curate any changes people care to submit, but I hope these just get forked and used either by companies that are trying to do the right thing by their employees and need help to get past their boilerplate policy docs, or by people who have management's ear and could use an example of how to make things better at work.
